600 patients (347 statin-free, 253 statin-treated), mean age 61.2 ± 9.8, with LDL cholesterol 100–220 mg/dl, triglycerides ≤ 600 mg/dl, a history of coronary heart disease (or equivalent, defined as diabetes, cerebrovascular disease or peripheral vascular disease) or estimated 1-year coronary heart disease risk > 20% |

Treatment regimen |
Atorvastatin, 10, 20, 40 or 80 mg/day, according to LDL-cholesterol level and status of statin use at baseline, titrated to next highest dose, if required, except for those allocated to 80 mg |
